ÖZET
Doğum sonrası dönem plasentanın doğumundan bir saat sonra başlar ve altı hafta kadar sürer. Bu dönemdeki kontrollerde hastanın tıbbi komplikasyonlar, emzirme, doğum sonrası depresyon, cinsel yaşam ve kontrasepsiyon açısından değerlendirilmesi annenin ve bebeğin sağlığı için önemlidir. Bu dönemde görülebilecek psikiyatrik rahatsızlıklar annelik hüznü, doğum sonrası depresyon ve doğum sonrası psikoz olarak sıralanmaktadır. Annelik hüznü hormonal değişikliklerce tetiklenen ve uykusuzluk, beslenme eksikliği ve yeni anne olmanın getirdiği stresle artan fizyolojik bir fenomen olarak değerlendirilir. Doğum sonrası psikoz genellikle doğumdan sonraki ilk iki hafta içinde manik, huzursuz davranışlarla kendini gösterir. Doğum sonrası depresyon ciddi sonuçlar doğurabileceği için erken tanı ve tarama önemlidir. Rutin taramada Edinburgh doğum sonrası depresyon ölçeği kullanılabilir. Tedavide kognitif yaklaşım ve ilaçlar etkilidir. Pek çok ilaç güvenli bulunmakla birlikte bebekte uzun dönemdeki potansiyel etkileri nedeniyle en düşük etkili doz kullanılmalıdır. Doğum sonrası psikoz intihar riskinin yüksek olduğu ciddi bir tablodur, tanı koyulduğunda psikiyatri uzmanına sevk edilmelidir.
ABSTRACT
The postpartum period begins one hour after delivery of the placenta and generally lasts six weeks. Oversight of four general categories (i.e.,medical complications, breastfeeding, postpartum depression, and sexuality and contraception) is vital to a mother's healthy recovery and her baby's healthy start. Psychiatric disturbances in the postpartum period are blues, postpartum depression and postpartum psychosis. The blues is considered a physiologic phenomenon triggered by hormonal changes and augmented by sleep deprivation, nutritional deficiencies and stress of new motherhood. Postpartum psychosis usually presents in the first two weeks postpartum as manic, restless behavior. Early recognition and screening for postpartum depression is important because of its' potentially serious consequences. Edinburgh postnatal depression scale can be used for routine screening. Management of postpartum depression may include cognitive and antidepressant therapy. Many drugs are found to be safe and effective but the minimum effective dose must be used for the potentially long term effects on the baby. Postpartum psychosis is a serious illness that is accompanied by suicidal ideation and the patient must be referred to the psychiatrists.
