Four Cases Of Twin Pregnancies Complicated With Single Fetal Demise

ÖZET
Amaç: Kliniğimizde izlenen, fetüslerden birinin ölümü ile komplike olan ikiz gebeliğe sahip 4 olgunun gebeliklerinin seyri doğumdan önce ve sonra annede gelişen komplikasyonlar, doğan çocukların büyüme ve gelişmelerinin incelenmesi amaçlandı. Materyel ve metod: 1995-1998 yılları arasında kliniğimizde izlem altında iken fetüslerden birinin ölümü ile komplike olan ikiz gebeliğe sahip 4 olguda fetüs; Doppler USG, NST, obstetrik USG ile değerlendirilir iken fetüsteki akciğer gelişimi amnion sıvısında lesitin /sfingomyelin oranları ölçülerek değerlendirildi. Bulgular: Bu özelliğe sahip 4 olguda da akciğer maturasyonu canlı bebekte sağlanıncaya kadar beklenilmişti. Bütün olgular için fetüs akciğer maturasyonu tespiti uygun gebelik haftalarındaki lesitin/sfingomyelin oranları ölçümlerine dayandırılarak yapılmıştı. Hemen hemen bütün bebeklerde hafif oranlarda dismatürite saptandı. Sadece bir bebekte doğum sonrasında ilk 24 saat içinde belirgin hipotoni, apne nöbetleri, sık tekrarlayan konvülziyonlar, multikistik ensefalomalazi ve sonrasında motor ve mental retardasyon gözlendi. Hiçbir annenin doğum sırasında ve sonraki izlemlerinde anormal bir bulguya rastlanılmadı. Sonuç: Yapılan retrospektif çalışmalar ve bizim çalışmamız ikiz eşlerinden birinin ölümüyle birlikte canlı kalan fetüsü ve anneyi pek çok riskin bekleyebildiğini hatta morbidite ve mortalite ile birlikte seyredebildiğini ortaya koymaktadır ancak önemli olan noktalar bu komplikasyonlar açısından yakın takip, agresif müdahalelerden mümkün olduğunca kaçınma ve akciğer matürasyonu tamamlanıncaya kadar yakın izlem olup sonrasında gebeliğin uygun şekilde sonlandırılması olarak özetlenebilmektedir.
ABSTRACT
Objective: The four cases who were treated in our clinic because of their complicated pregnancies with single fetal death were included into the study with the aim of investigation by means of possible maternal complications that can be seen before or after the birth and the course of development and growth of the fetus. Materials and Methods: The four fetuses in four cases who were treated in our clinic between the years 1995-1998 because of their complicated twin pregnancies with single fetal death were investigated with Doppler USG, NST, obstetrical USG and the developmental status of lungs of fetusus were investigated with Lecithin/sphingomyelin analyses in the amniotic fluid. Results: All of four cases were waited until the achivement of lung maturation in the single alive fetus. The detection of lung maturation were performed with the lecithin/sphingomyelin analyses in all cases. Mild dismaturity was observed in nearly all fetusus. In only one fetus, hipotony, apnea attacks, frequent convulsions, multicystic encephalomalasia; during for first 24 hours, and subsequently motor and mental retardation were detected. In the perinatal or postnatal follow ups of all mothers no any anormal finding were found out. Conclusion: The available studies and our study have been revealing that in the twin pregnancies with the complication of single fetal death, both fetuses and mothers are canditates for many risks including morbidity and mortality but regarding this subject the most important points can be summarized as; intensive follow-up because of possible complications, avoiding aggresive interventions, waiting until the maturation of fetal lungs, and subsequent appropriate termination of pregnancy.
