ABSTRACT
Objective: To investigate the relationship between kinesiophobia levels and rectus femoris (RF) muscle architectural parameters, pain and functional performance in individuals with knee osteoarthritis (OA). Material and Methods: 98 women aged 45-65 years with knee OA were included in the study. Individuals' fear of physical activity was assessed with the Tampa Kinesiophobia Scale. B-Mode ultrasonography was used to examine the muscle thickness and pennation angle of the RF muscle. Visual analogue scale was used to evaluate knee pain. Repetitive sit-to-stand test and Knee Injury and Osteoarthritis Outcome Score (KOOS) were used for functional performance evaluation. For the statistical analysis the Pearson, the Spearman correlation test and basic linear regression analysis were used. Results: There was a moderate negative correlation between kinesiophobia and RF muscle thickness (r=-0.471, p=0.009) and a weak negative correlation with pennation angle (r=-0.307, p=0.026). There was a moderate positive correlation between kinesiophobia and pain (r=0.639, p=0.001). There was a moderate positive correlation between kinesiophobia and repetitive sit-to-stand test (r=0.538, p=0.001). In addition, kinesiophobia was negatively correlated with each sub-parameter of KOOS (p<0.05). Regression analysis revealed that pain explained 39% of the variance in kinesiophobia, RF muscle thickness 12%, RF pennation angle 2%, and repetitive sitting and standing 14%. Conclusion: This study showed that kinesiophobia was associated with RF muscle thickness and pennation angle, pain and functional tests in women with knee OA. It was found that RF muscle thickness and pennation angle indicating force generating capacity decreased in individuals who had fear of movement and avoided activity due to kinesiophobia.
