After exclusion of those patients with systemic diseases that affect pregnancy outcomes, cervical cerclage, and fetal anomalies, the remaining study group of 2170 women and the control group consisting of 2046 women were evaluated according to pregnancy complications such as abortion, preterm birth, preeclampsia and intrauterine growth retardation. Results: The correlations between low PAPP-A level (≤0.5 MoM) and preeclampsia (p<0.001), intrauterine growth failure (p< 0.001), preterm birth (p< 0.001), gestational diabetes mellitus (p= 0.040), oligohydramnios (p= 0.003), spontaneous abortus (p= 0.003), still birth (p< 0.001), were statistically significant. No statistically significant correlations were found between low PAPP-A level (≤0.5 MoM) and placenta previa (p= 0.496) and placental abruption (p= 0.110). Conclusion: Significant correlations were found between low PAPP-A level, and many pregnancy complications. But, PAPP-A alone remains insufficient in routine clinical evaluation of pregnancy complications because of its limited sensitivity.">

Association Between Low Levels of Maternal Serum PAPP-A in the First-Trimester and Complications of Pregnancy

ÖZET
Amaç: İkili tarama testinin bir bileşeni olan, maternal serum "pregnancy associated plasma protein-A (PAPP-A)" değerleri ile gebelik komplikasyonları arasındaki ilişkiyi saptamak ve maternal serum PAPP-A MoM değeri ile gebelik komplikasyonlarının öngörülebilirliğini araştırmaktır. Gereç ve Yöntemler: Bu çalışmada, hastanemiz antenatal polikliniğinde ikili tarama testi ve sonrasında diğer gebelik tetkik ve takipleri yapılan hastaların olumsuz gebelik sonuçları, PAPP-A düzeylerine göre değerlendirildi. İkili tarama testi yapılan 15,886 gebeden, PAPP-A değeri 0,5 MoM ve altında olan 2385 gebe çalışma grubu olarak alındı. Aynı tarihler arasında, PAPP-A değeri 0,5 MoM'un üzerinde olan 2210 gebe kontrol grubu olarak alındı. Gebelik sonuçlarını etkileyebilecek sistemik hastalığı olan, servikal serklaj yapılan ve fetal anomalisi olan gebeler dışlandıktan sonra geriye kalan 2170 gebeden oluşan çalışma grubu ve 2046 gebeden oluşan kontrol grubu, abortus, preterm eylem, preeklampsi, intrauterin gelişme geriliği gibi gebelik komplikasyonları açısından değerlendirildi. Bulgular: Düşük PAPP-A düzeyi (≤0,5 MoM) ile preeklampsi (p<0,001), intrauterin gelişme geriliği (p<0,001), preterm doğum (p<0,001), gestasyonel diabetes mellitus (p=0,040), oligohidroamnios (p=0,003), spontan abortus (p=0,003), ölü doğum (p<0,001) arasındaki ilişki istatistiksel olarak anlamlı bulunmuştur. Düşük PAPP-A düzeyi (≤0,5 MoM) ile plasenta previa (p=0,496) ve plasenta dekolmanı (p=0,110) arasında ise anlamlı bir ilişki bulunamamıştır. Sonuç: PAPP-A düzeylerinin düşük olması ile birçok gebelik komplikasyonu arasında anlamlı bir ilişki bulunmuştur. Fakat yeterli duyarlılığı olmadığı için, gebelik komplikasyonlarının rutin klinik taramasında, tek başına PAPP-A yetersiz kalmaktadır.
ABSTRACT
Objective: To investigate whether maternal serum values of pregnancy associated plasma protein-A (PAPP-A), a component of first trimester screening test, are associated with pregnancy complications and to determine its value in predicting pregnancy complications. Material and Methods: In this study, the adverse pregnancy outcomes of the women who underwent first trimester screening test and had pregnancy follow-up in the antenatal clinic of our hospital, were evaluated according to PAPP-A values.
