Impacts of the COVID-19 Pandemic on Adults with Non-Communicable Diseases: Cross-Sectional Study in an Urban Area

ÖZET
Amaç: Bu çalışmada, kronik hastalıkları olan erişkinlerin koronavirüs hastalığı-2019 [coronavirus disease-2019 (COVID-19)] pandemisi döneminde hastalık yakınmalarındaki değişimin, pandemi döneminde algılanan genel sağlık ve ruh sağlığı değişimlerinin ve ilişkili faktörlerin değerlendirilmesi amaçlanmıştır. Gereç ve Yöntemler: Mart-Temmuz 2022 tarihleri arasında Manisa il merkezinden seçilen 10 aile sağlığı merkezlerine başvurulara dayanan kesitsel bir çalışmadır. Veriler araştırmaya katılan 500 kişiden yüz yüze toplanmıştır. Araştırmanın bağımlı değişkenleri COVID-19 pandemisi döneminde kronik hastalık semptomlarındaki değişim ve pandemi dönemindeki algılanan genel sağlık ve ruh sağlığı değişimidir. Veri analizinde SPSS 23.0 kullanılmıştır. Çok değişkenli analizlerde lojistik regresyon uygulanmıştır. Bulgular: Katılımcıların yaş ortalaması 60,1±8,4 yıldır. Çalışmamızda katılımcıların %51,8'inin pandemi döneminde kronik hastalıklarıyla ilgili yeni yakınma/yakınmalarında artış olmuştur. Katılımcıların %37,2'si genel sağlık durumunun, %23,2'si ruh sağlığı durumunun pandemi döneminde öncesine göre daha kötü/çok daha kötü olduğunu belirtmiştir. Çalışmamızda, pandemi döneminde sağlık hizmetlerine ulaşmada sorun yaşayan kişilerde, kronik hastalık risk faktörleri olumsuz yönde değişime uğrayan kişilerde, ilaç tedavisine uyumlu olanlarda ve kronik hastalık bakım niteliği pandemi döneminde olumsuz etkilenenlerde hastalıkla ilgili yeni yakınma/var olan yakınmalarda artış riski artmıştır. Pandemi döneminde algılanan ruhsal sağlık değişimi açısından kadınlar, uyku düzeni bozulanlar, aile içi ve sosyal ilişkileri bozulanlar daha risklidir. Çalışmamızda, pandemi döneminde sağlık hizmetlerine ulaşmada sorun yaşayan kişilerde ve kronik hastalık bakımı olumsuz etkilenen kişilerde algılanan genel sağlık değişiminin kötü olma riski artmıştır. Sonuç: Kronik hastalığı olan bireylerin yaklaşık yarısının pandemi döneminde hastalık semptom ve yakınmalarında artış/ yeni yakınmaları olmuştur. Kronik hastalık bakım niteliğinin COVID-19 pandemisi döneminde zayıfladığı görülmüş; bulaş korkusu, randevu alamama ve kısıtlamalar nedeniyle hastaların rutin kontrolleri aksamıştır.

ABSTRACT
Objective: It is the evaluation of the change in the complaints of the disease during the coronavirus disease-2019 (COVID-19) pandemic of chronic diseases, the perceived general health and mental health changes and the associated factors. Material and Methods: It is a cross-sectional study based on applications for ten family health centers selected in Manisa between March and July 2022. Data were collected face to face from 500 people who participated in the research. Dependent Variables The change in the symptoms of chronic disease during the COVID-19 pandemic period is the general change of general health and mental health. SPSS 23.0 was used in data analysis and logistics regression was applied. Results: The average age of the participants is 60.1±8.4 years. 51.8% of the participants had an increase in complaints related to their chronic diseases during the pandemic period. 37.2% of the participants stated that their general health status and 23.2% stated that their mental health status was worse than before. There is a higher risk of increasing symptoms related to the disease in people who have problems in achieving health services, in people who have changed negatively in chronic disease risk factors, those who are compatible with drug therapy, and those who weaken chronic disease care. Women, sleep patterns deteriorate, family and social relations are more risky for mental health. The perceived general health of those who do not reach health care and weakened chronic disease care has deteriorated. Conclusion: Approximately half of individuals with chronic diseases experienced an increase in disease symptoms and complaints/new complaints during the pandemic period. The quality of chronic disease care has weakened during the COVID19 pandemic, and routine check-ups of patients have been disrupted due to fear of contamination, inability to make appointments and restrictions.

1. TERMS OF USE

1.1. 4.2. People benefiting from the services provided by "Turkiye Klinikleri" and using the website can use the "SITE" only according to the law and only for personal reasons.
