Prostat kanseri; ülkemizde ve dünyada ilk 3 sırada yer alan heterojen bir kanserdir. 1980'li yıllarda prostat spesifik antijenin (PSA) kullanılmaya başlaması ile erken tanı ve tedavisi başarıyla yapılabilmektedir. Günümüzde standart biyopsi tekniklerine ek olarak füzyon biyopsisi ve manyetik rezonans görüntüleme kılavuzluğunda kognitif biyopsi, prostat kanseri tanısı için önemli tanı yöntemleri haline gelmiştir. Biyopsi ile tanısı konulan kanserin oldukça etkin tanı yöntemleri ile günümüzde doğru evrelemesi yapılabilmektedir. Özellikle PSMA-PET/BT, hastalığın tanı ve evrelemesinde çığır açmıştır. Böylece kansere yönelik uygulanan daha etkin tedaviler sayesinde hastanın yaşamını uzatıp, hayat kalitesini iyileştirme olasıdır.
Oligometastatik prostat kanseri tanımı; çoğunlukla metastazların sayısı ve yeri ile tanımlanan, belirgin bazı tanı ve evrelendirme özelliklerine sahip sınırlı metastaz içeren yaygın tümörlerden biyolojik olarak farklı görünen, heterojen bir hastalık grubunu içerir. Bu hastalar; primer tümörü tedavi edilmemiş, senkron metastaz gelişen ve metastaz bölgesi beş veya daha az olan bir hasta grubunu oluşturur. Oligometastatik prostat kanserinde yukarıda bahsedildiği gibi tanı ve tedavide teknolojik gelişmelere bağlı olarak yapılacak multimodal tedaviler sınırlı metastatik yüke sahip bu alt grup için önemli bir sağkalım faydası göstermektedir. Metastazlara yönelik tedavi ve sitoredüktif prostatektomi hakkındaki son veriler umut verici sonuçlar göstermektedir ancak hâlâ prospektif çalışmalar eksiktir ve bu nedenle deneysel ortamlarla sınırlı olmalıdır. Primer tümör varlığında, primer tümör ve oligometastazlar lokal tedavi edilirse, uzamış hastalıksız sağkalım avantajı, hatta kür sağlanabilmektedir. Hastalığın tedavi planlamasında mutlaka multidisipliner yaklaşım gereklidir.
Türkiye Klinikleri'nin bu kitabında çok tartışmalı bir konu olan oligometastatik prostat kanseri, klinik uygulamaya yönelik güncel literatür eşliğinde multidisipliner olarak sunulmuştur. Bu kitabın amacı, oligometastatik prostat kanseri tedavisine ilişkin mevcut verileri özetlemek ve tartışmaktır. Kuşkusuz bu konuda daha fazla prospektif ve randomize çalışmalara gereksinim vardır. Bu kitabın hazırlanmasında emeği geçen ve katkıda bulunan tüm meslektaşlarıma özverileri için teşekkürü borç bilirim. Prostate cancer is a heterogeneous malignancy that ranks among the top three most common cancers both in our country and worldwide. Since the introduction of prostate specific antigen (PSA) testing in the 1980s, early diagnosis and treatment have been successfully achieved. In addition to standard biopsy techniques, fusion biopsy and cognitive biopsy guided by magnetic resonance imaging have become important diagnostic methods for prostate cancer. With the advancement of highly effective diagnostic techniques, accurate staging of biopsy-confirmed cancer is now possible. In particular, PSMA-PET/CT has revolutionized the diagnosis and staging of the disease. Consequently, more effective cancer-targeted treatments have the potential to prolong patient survival and improve quality of life.
The concept of oligometastatic prostate cancer encompasses a heterogeneous group of diseases that appear biologically distinct from widely metastatic tumors, typically defined by the number and location of metastases and specific diagnostic and staging characteristics. This patient group consists of individuals with untreated primary tumors who develop synchronous metastases, with five or fewer metastatic lesions. As mentioned above, technological advancements in diagnostics and treatment have led to the development of multimodal therapeutic strategies, which offer significant survival benefits for this subgroup with limited metastatic burden. Recent data on metastasis-directed therapy and cytoreductive prostatectomy have shown promising results; however, prospective studies are still lacking, and therefore, such approaches should remain within experimental settings. In cases where the primary tumor is present, if both the primary tumor and oligometastases are treated locally, prolonged disease-free survival, and even potential cure, may be achieved. A multidisciplinary approach is essential in treatment planning for this disease.
In this book by Türkiye Klinikleri, the highly debated topic of oligometastatic prostate cancer is presented in a multidisciplinary manner, accompanied by contemporary literature relevant to clinical practice. The aim of this book is to summarize and discuss the existing data on the treatment of oligometastatic prostate cancer. Undoubtedly, further prospective, randomized studies are needed in this field. I extend my sincere gratitude to all my colleagues who contributed to and supported the preparation of this book. I would also like to thank all the staff of Türkiye Klinikleri for their dedicated efforts in organizing the book, facilitating communication with faculty members, and overseeing the publication process.
